The approach
Primary care should be more than a quick visit when you are sick. As a board-certified adult-gerontology nurse practitioner, Ismelis provides ongoing care for adults, with time to understand your history and the conditions you live with.
We manage common chronic conditions such as high blood pressure, high cholesterol, prediabetes and thyroid disease, and we go further: looking for what drives them and building habits that prevent the next problem.

FAQ · Primary & Preventive Care
Questions, answered
01Can a nurse practitioner be my primary care provider?
Yes. Nurse practitioners are licensed to diagnose, treat, order tests and prescribe medication. Ismelis Gata is a board-certified adult-gerontology nurse practitioner (AGNP-C) licensed in Florida.
02Can I get primary care by telehealth?
Much of adult primary care works well by telehealth: reviewing symptoms and labs, managing chronic conditions, adjusting medications and prevention planning. When you need a physical exam, you can see Ismelis in person at our Coral Gables office; for an urgent test, we will direct you to the right place.
03Do you see children?
No. We care for adults age 18 and older who live in Florida.
04Which labs are included in a preventive checkup?
Beyond the usual blood count and metabolic panel, we often look at fasting insulin, HbA1c, an advanced lipid panel, a full thyroid panel, vitamin D, B12, iron and inflammation markers, chosen for your age and risk.
05Where do I get my blood drawn?
At a local lab of your choice in Florida. We send the order, you visit the lab at a convenient time, and we review the results together in your follow-up.
06Can you manage my blood pressure or cholesterol medication?
Yes. We can manage common chronic conditions, including medication, and we also work on the habits and root causes that may let you need less over time, always safely and with monitoring.
07What if I need a specialist or urgent care?
We coordinate referrals to specialists when needed. For urgent symptoms, go to urgent care or the emergency room, and for emergencies call 911.
