Getting started
01What can I expect in my first visit?
Your initial consultation includes a full symptom review, a discussion of your health goals, and lab ordering if appropriate. It can be by telehealth or in person at our Coral Gables office, and you will leave with clear next steps.
02Is this service available outside Florida?
Currently we only serve adult residents of Florida, age 18 and older. You must be located in Florida at the time of each visit.
03Are visits in person or online?
Both. You can choose a telehealth visit by secure video or an in-person consultation at our office in Coral Gables. Lab work is done at a local lab of your choice, and medications and supplements can be delivered to your home.
04Who will I see?
You will see Ismelis Gata, MSN, APRN, AGNP-C, a bilingual board-certified nurse practitioner with more than 18 years of medical experience. The same provider follows your care from the first visit on.
05Do you offer visits in Spanish?
Yes. Ismelis is fully bilingual, so your visits can be in English or Spanish, whichever you prefer.
06How do I book an appointment?
Choose a time in our online scheduler. New patients then complete intake forms in the secure patient portal before the first visit.
07Is this a good fit for emergencies or urgent problems?
No. This practice is not for emergency medical services. If you are experiencing an emergency, call 911, or the Suicide & Crisis Lifeline at 988.

Cost and insurance
01Do you accept insurance?
No. Services are cash-based. We do not accept insurance at this time.
02Why is the practice cash-based?
It lets visits be as long as they need to be and lets care follow what you need, not what a plan approves. You know the cost of each visit before you book.
03Are lab tests or supplements included in the visit cost?
No. Lab testing and supplements are additional. We guide you through the options based on your personalized plan, and always explain costs first.
04Can I use my HSA or FSA?
Many patients use HSA or FSA funds for medical visits and lab work. Eligibility depends on your plan, so check with your plan administrator.
05Can I ask for a superbill?
If you want to request out-of-network reimbursement from your insurer, ask us for a receipt with the information insurers usually need. Reimbursement is not guaranteed and depends on your plan.
06Can I use my insurance for lab work?
In some cases labs can go through your insurance at the lab; in others a cash lab price is lower. We help you choose the most practical option.
07Is functional medicine covered by Medicare?
We do not bill Medicare or other insurance. Medicare patients can still receive care as private-pay patients, according to the rules that apply to them.

Labs, medications and supplements
01How are medications or therapies delivered?
Medications and supplements, if applicable, are delivered directly to your home through trusted partner pharmacies and Fullscript.
02Where do I do my lab work?
At your preferred local lab in Florida. We send the order electronically, and some specialty tests are kits you complete at home.
03Do I need to fast before my blood test?
For many metabolic panels, yes, usually 8 to 12 hours with water only. Your lab order will say exactly how to prepare.
04What is Fullscript?
Fullscript is an online dispensary for professional-grade supplements. Through our store you get the exact products in your plan, with auto-refill options and direct shipping to your home.
05Do I have to buy supplements from you?
No. Our Fullscript store is a convenience that guarantees quality and the right dose. You are free to buy equivalent products elsewhere.
06Can you prescribe medication?
Yes. As a nurse practitioner licensed in Florida, Ismelis can prescribe when medication is appropriate, and it is sent to a partner or local pharmacy.
07Will you work with my other doctors?
Yes. With your permission we share results and plans with your other providers, so your care stays coordinated.
